Scouting Medical Forms Required!

Any person participating in any Order of the Arrow activity must have a current medical form on file with the Lodge.

Please complete parts A and B and bring a copy of the form with you to the next meeting or event.

Forms must be updated every year, so if you don’t remember bringing one this year, bring it to the next event. Individuals who do not have a medical form on file will be required to complete one on-site or leave the event.
